Community Care Collaborative Identifies County Hypertension Rates

The 6 largest primary care practices in Lancaster County saw 70% of the county’s adult patients last year. Analysis of practice data shows that about 45,000 (or 17%) of those patients do not have their blood pressure under control. LiveWELL Lancaster County

LiveWELL Lancaster County is the name of the five-year Lancaster County Community Transformation Grant program. Funded through a grant from the Centers for Disease Control and Prevention, LiveWELL Lancaster County initiatives are designed to encourage healthier lifestyle choices and promote better health among community residents. These activities are integrated across five strategic areas:
